We also have a list of documents on our website under the FOIA heading (in addition to this policy). They include a direct link to the FOIA email, the Categories of Public Records, Library Information Director, Organizational Chart, Records Request Form (not that they have to use this), and the link to the Board of Trustees.
In addition, our Policy Manual which contains Board approved policies addresses library card eligibility with this language: Eligibility Residents of the Library District, nonresidents who own taxable property within the district, and Library employees are eligible to receive a library card at no cost.
We issue library cards to residents in a local nursing home according to the same library card policy we apply to other patrons, but instead of asking the individual resident for proof of address we have the nursing home staff verify they are residents in the nursing home.
Our resolution is attached along with the explanatory cover sheet we gave our Board when recommending it. It was written in-house and passed by our board before samples were available from Ancel Glink, et al. We did run it by our attorney before passage. Rather than state a specific per diem rate which would need to be updated regularly, it refers to the IRS allowable rates. Our business office checks rates before I authorize overnight travel.
